There are many ways to get involved with students, in classrooms and at buildings within Peoria Public Schools. To maintain student safety, all volunteers are required to complete a background check.

You can complete the form by downloading it here, then emailing or scanning the signed form to HRSTAFF@PSD150.ORG. Physical forms can also be dropped off at the Administration Building, located at 3202 N Wisconsin Ave, Peoria, IL 61603.
