District New User Setup Form (staff logins only)

This is only needed for non-district employees (such as contractors) who need access. Any such request must be approved by HR. Used for adding a new non-district user to the district network systems. Must be signed by either the Director of HR or the school's Executive Director. (For district employees, contact HR and they will submit the information electronically to technology via Skyward.)

Technology Needs for Professional Development Activity (staff logins only)

This form is used when you create a help desk ticket and select the Professional Development Setup request type.
