Below is the Opt Out Form for the mandatory testing COVID-19 testing program for Peoria Public Schools high school students. The purpose of this testing program policy, which was passed by the Board of Education, is to mainatin a safe environment in our schools by detecting any asymptomatic cases in our high school buildings that would otherwise go undetected.
NOTE: Opting out of the COVID-19 testing program means that a student will not be able to participate in in-person instruction or extracurricular activities during the rest of the 2020-2021 school year.
As a reminder, the purpose of this form is for any parent/guardian who does not want their student to undergo the periodic testing, which will result in the student not being able to participate in in-person learning and will require at-home learning as a result.
